What is a sports physical?
A sports physical, also called a preparticipation evaluation, reviews health history and physical health in relation to sports participation. It can identify questions that need more evaluation before an athlete is cleared. It is not a guarantee of clearance and does not replace a regular preventive health visit.
What to bring to QuickDoc
Call your clinic to confirm age eligibility, consent requirements and whether the requested form can be completed. Complete the history portion of your form in advance if your program asks you to do so.
- Your school or team’s current physical form and instructions.
- A photo ID and parent or guardian information, when applicable.
- A list of medicines, allergies, past injuries and relevant health conditions.
- Glasses or contact lenses if you normally use them.
Plan before the deadline
Leave time before the first practice or registration deadline. A clinician may need additional records or recommend follow-up before completing clearance. Ask your school or league how recent the examination must be and where to submit the finished form. Keep a copy for your records.
A clear price for the physical
Sports physicals are free at Warren. At other QuickDoc locations, the listed price is $20 per physical, not a weekly fee. If another service is recommended, ask about its cost before proceeding. Contact an open QuickDoc clinic for current availability; Warren is now open. Ypsilanti is coming soon.
